As part of a national NHS scheme five areas were chosen which the Primary Care Networks (that’s us!) are responsible for and which we need to make plans on what we will do to improve on these areas. Below are the five areas and a brief outline of our plans:
- Leadership – to get a solid foundation for where we want to be at the end of each year and who is responsible for each aspect of the plans.
- Population Health – to use data to help us make the best decisions for our patients, especially those who are vulnerable, leading to proactive care in the community for every individual.
- Integrating Care – to work across the sectors to ensure all manner of primary care is available, being part of a Primary Care Network makes this much easier and allows us to reach a greater population with these services.
- Managing Resources – to ensure we direct patients to the right service for them, this helps the patient to receive the best possible care while keeping waiting lists shorter for everyone and freeing up our GPs for those who need them.
- Working with people and communities – to contact local community groups and raise awareness of the Primary Care Network with the aim of understanding and serving the needs of our population better than ever.

Chester-le-Street Primary Care Network consists of 7 GP practices working together to improve the health of our population.
